DownloadThese are the results of the DBHS Client Survey Form answers submitted. The data provides the average score of each answer, ranging from 1 (Poor) to 5 (Excellent). The averages are calculated based on the total score received per survey answer divided by the total number of families that submitted an answer to the relative question. The overall average score is determined as either Poor, Below Average, Average, Above Average, or Excellent.
DBHS requests that each family that receives services from DBHS submit one collective survey to include in the quarterly assessment of DBHS services provided.

These are the results of the DBHS Annual Employee Survey Form submitted. The data provides the average/general response of each answer. The averages are based on the total score or general response received per survey answer considering the total number of employees that submitted an answer to the relative question. The overall "average" response is evaluated as either Needs Work, Acceptable, or Exceptional. Any score provided in parenthesis is the total value submitted. (Note: Yes=2, No=1)
DBHS requests each active employee to submit a DBHS Annual Employee Survey to assess our operations, practices, and values from an internal perception. DBHS Management reviews these survey responses and determines what Needs Work (NW), is Acceptable (AC), or is Exceptional (EX). We may request an annual survey from an inactive/ex employee that had involvement with DBHS operations within the last six months prior to the survey submission period (June to December).
